After all required sections are filled out, a blue ‘Click to Sign’ button will appear at the bottom of the document. The forms will not be submitted to our office unless the button is pressed. Please call our office if you are having any issues with the online forms. If you would prefer to fill them out in then office, we kindly ask that you arrive 10 minutes prior to your scheduled appointment time to allow for adequate time. Please bring any updated medication lists that you may have.
